The VaxYes portal requires a full match to the first name, last name, mobile number and date of birth you used to register with. Some common reasons for mismatch:

  • data entry error

  • using a nickname or middle name where a full name should be

  • adding/omitting initials, prefixes, suffixes (Jr., Sr, etc.)

  • date of birth mismatch

  • unable to receive verification code (text START to 80578)

If you believe that your access is limited due to the reasons listed above, please reattempt your VaxYes submission with new photos. Be sure they are clear, well lit and in focus. Be sure that the spelling, names, and date of birth used in your submission match your government issued ID.
